Tournament Background:
We are pleased to invite you to the 2015 Top of Texas Karate Championships to be held on 19 September in the Amarillo Civic Center Heritage Room, Amarillo Texas.

This year’s tournament is sanctioned by the USA National Karate Federation (USANKF) and will be using slightly modified World Karate Federation (WKF) rules.

As in previous Top of Texas Karate Championships it is our goal to promote friendship and goodwill through fair, balanced, and friendly competition among traditional Japanese / Okinawan karate practitioners. All traditional Japanese / Okinawan Karate dojo are welcome to attend so please feel free to share information about this event with your friends and colleagues.

Medical Release:
By clicking the "I Agree" you are electronically signing this agreement indicating that you have read, and understood the terms and conditions, refund policy, and release of liability, you are 18 years of age or older or the parent or legal guardian of the minor registering for this tournament or event. In the case of registering a minor you attest that the name entered as the parent or legal guardian is the name of the person accepting the agreement and you are in fact the parent or legal guardian of the registered minor.

**WAIVER AND RELEASE OF LIABILITY**

I the undersigned, in consideration for being permitted to participate in the 2015 Top of Texas Traditional Karate Championships:

1. Acknowledge, agree, and represent that I understand the nature of the activity and that I am qualified, in good health, and in proper physical condition to participate in such
activity. I further agree and warrant that is at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the activity.

2. Fully understand that: a) There are risks and dangers associated with participation in martial arts events and activities which could result in bodily injury, partial and/or
total disability, paralysis, or death; b) the social and economic losses and/or damages, which could result from these risks and dangers described above could be severe;
c) These risks and dangers may be caused by the action, inaction, or negligence of others participating in the activity including, but not limited to, the Releases named below;
d). There may be other risks not known to us or are not reasonably foreseeable at this time.

3. Participant (or if a minor, parents or legal guardian of the minor) accepts the risks and dangers, and assumes all responsibility for any losses, costs, and damages incurred,
if any, while attending and/or participating in this event.

4. Participant or Parent/Legal Guardian of a minor participant hereby waives, discharges, and promises not to sue the Amarillo Martial Arts Center, Lee Gray, the Amarillo
Civic Center, the City of Amarillo, all sponsors, judges, instructors, coaches, other participants, volunteers, administrators, managers, employees, and/or all other individuals
involved in any way whatsoever with the planning, production, promotion, and operation of the 2015 Top of Texas Traditional Karate Championships and hereby releases
and forever holds harmless from any liability arising from participation in the afore mentioned event.

5. I fully understand that any medical treatment provided will be of a first aid nature only.

6. The individual signing further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted
by the law of the jurisdiction in which the event is conducted and that if any portion is held invalid, it is agreed that the balance shall, notwithstanding, continue in full
force and effect.
