By clicking the "I Agree" you are electronically signing this agreement indicating that you have read, and understood the terms and conditions, refund policy, and release of liability, you are 18 years of age or older or the parent or legal guardian of the minor registering for this tournament or event. In the case of registering a minor you attest that the name entered as the parent or legal guardian is the name of the person accepting the agreement and you are in fact the parent or legal guardian of the registered minor.

Notice: this agreement contains indemnity and release provisions. In submitting this application, I state that I am physically able to participate in the above mentioned karate tournament and have not been directed by a medical authority against an activity of this type. I also understand that I hereby assume full responsibility for any and all damages, injuries, or losses that I may sustain or incur while attending, traveling to and from, or participating at this event. I hereby release, and waive all claims against, James Lee Karate Inc., James Lee, Owners of building of premises in which the event is held and all other members, judges, referees, arbitrators, and participants of the event from liabilities of this nature. I fully understand that under no circumstances will any refund of entry fees or charges be assessed.
The undersigned (or undersigned’s legal guardian) agrees that any photos or videos taken of the undersigned during this event may be used
by James Lee’s Karate in its promotional materials or on its website with no compensation or other remuneration due to the undersigned
and the undersigned hereby waives any claims they may have against James Lee’s Karate for the use of such photos and videos.
