From Portland
When traveling South on I-5, take exit #294 for Tigard / McMinnville and follow the signs to McMinnville. Continue on Highway 99W through Dundee until the Highway 18 “Ocean Beaches” junction. Turn left on Highway 18 and continue for approximately four miles. The Museum is on the right.

From Salem
When traveling North on I-5, take exit #253 at Salem for Highway 22 / Mission Street and follow the signs for Highway 22 / Mission Street through Salem. Stay to the left on Highway 22 after crossing the Willamette River. Turn right (north) at the intersection with Highway 99W. Continue northbound on Highway 99W to the outskirts of McMinnville. Take the Highway 18E exit and continue for approximately four miles. The Museum is on the left.

From the coast
When traveling on Highway 101, take Highway 18 and continue until the “Y” just before entering the City of McMinnville. Stay to the right at the “Y” heading toward Portland and Dayton. Continue on Highway 18 for approximately five miles. The Museum is on the left.

From Portland International Airport (PDX)
Travel east on Airport Way; follow signs for and merge onto I-205 S. After 3 miles, take exit 21B for I-84 W and continue for 5 miles. Use the left two lanes to take the exit towards Salem, then merge onto Interstate 5 South and follow directions above to continue on to McMinnville.

RELEASE AGREEMENT - READ CAREFULLY BEFORE SIGNING
In consideration of being allowed participation in any way in the Saito Karate Do Classic and related events and activities, the undersigned:

ITEM 1. Agrees that prior to participating the participant and/or Parent or Guardian (if under 18), will inspect the facilities and equipment to be used, and if the participant and/or parent or guardian, believes anything is unsafe, they will immediately advise the tournament director of such condition(s) and refuse to participate.

ITEM 2. Acknowledges and fully understands that each participant may be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their action, inaction or negligence but the actions, inaction or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, the participant, and/or parent or guardian acknowledges that there may be other risks not known or not reasonably foreseeable at this time. The participant and/or parent or guardian assumes all the foregoing risks and accepts personal responsibility for the damages following such injury, permanent disability or death.

ITEM 3. Releases, waives, discharge and covenants not to sue the event organizer, the Mendonca Academy of Martial Arts, LLC, the Traditional Karate Federation International USA, LLC, its officers, its affiliated clubs, staff, officials, volunteers, regional sports organizations, their respective administrators, directors, agents, coaches and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessees of premises used to conduct the event, all of which are hereinafter referred to as “releases” from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise.

ITEM 4. All entries are final; no refunds will be given. I fully understand that any medical treatment given will be of a first aid treatment type only. I consent that any pictures furnished by me or any pictures taken of me in connection with the tournament can be used for publicity, promotion or television showing now or in the future, and I waive compensation in regard thereto. All participation in any event or class in this tournament is by permission only. The Director or his authorized agent(s) reserve the right to refuse entry to any person, school, team or club.

ITEM 5. Please Note: All participants must be covered by health or medical insurance in order to participate.

ITEM 6. Statement of Health: By my and/or Parent/Guardian’s signature below I confirm that I am in sound health and there is no reason why I cannot participate in this event. I also confirm, to the best of my ability that the participant is Covid19 Symptom free. Furthermore I agree to present a PCR test or my Covid vaccination card the day of the event during registration.

ITEM 7. The undersigned has read the above waiver and release, understand that they have given up substantial rights by signing it and sign it voluntarily. If under 18 years of age, the approval of a parent or legal guardian is required.
I, the undersigned, have read and specifically understand the above release and agree to be bound by its terms on behalf of my spouse, my child, and myself.
