I the undersigned, do hereby submit my application for participation in the 32nd Annual Sacramento Karate Championships at Cosumnes River College on Saturday June 7. 2025 and hereby assume full liability for all damages, injuries, or losses that I may sustain or incur while participating and hereby waive all claims against the promoters, operators, sponsors, and/or officials of the Sacramento Karate Championships for any injury that I may sustain. I fully understand that any medical treatment given will be of a first-aid nature only.
